The &lt;a href="https://www.idf.org/sites/default/files/attachments/article_593_en.pdf"&gt;North Karelia Project&lt;/a&gt; was launched in 1971 to address this situation. Prevention was chosen as the most effective public health solution. "The general idea was to transform the social and physical environment of North Karelia. After an initial 5-year period, the project was extended locally, and its findings diffused nationwide to promote prevention throughout Finland. An integrated approach to prevention was central to success of the project. The risk factors for cardiovascular disease-unhealthy diet, smoking and physical inactivity-are risk factors for many chronic diseases. Rather than a number of vertical disease specific programs, an integrated approach targets the main common behavioral risk factors." Thinking advanced from a focus on risk factors for individuals into a broader emphasis upon the needs for society, environment and community to be supportive for health and to enable healthy living, i.e. a more comprehensive approach based on health determinants. &lt;a href="http://ctb.ku.edu/en/dothework/northkareliaproject.aspx"&gt;Social marketing&lt;/a&gt; was an important aspect of the North Karelia Project. Dramatic changes were observed in risk-related lifestyles and risk factors in the people of North Karelia and this was associated with substantial reduction in population-wide rates of cardiovascular disease. With the nationwide roll-out of activities, the prevalence of risk factors and death due to cardiovascular disease began falling throughout Finland. &lt;br /&gt;&lt;br /&gt;The North Karelia Project was adapted by cardiologists at Mayo Clinic for implementation in &lt;a href="http://www.healthylivingrochester.org/images/uploaded/4%20year%20paper.pdf"&gt;Olmsted County, Minnesota&lt;/a&gt; and in 2002 it served as a model for initiating Healthy Greenville 2010, which has evolved into&lt;a href="http://livewellgreenville.org/"&gt; LiveWell Greenville&lt;/a&gt;. Communities with similar public health projects are New York City, Savanah, Louisville, Oklahoma City and Chicago. Reviewing the past is often done to put a fine point on progress but I do not look back to measure progress. Orientation and direction are what I seek. It is a matter of what and why more than where, when and how much or how far, but it is a road map nonetheless. Progress is of little value if it is not in the correct or desired direction, as well as the right distance or amount. When the unknown exceeds the known, as it so often does, process occupies the center of attention with the expectation that process, properly executed, will lead to a designated goal. But the value of process is determined by periodic evaluation of orientation and direction. It is easy to be lost and not know it.&lt;br /&gt;&lt;br /&gt;Looking back for orientation and direction can refresh the wisdom gained from experience and cultivate the humility that comes with recognition of life as a mystery. The correct or desired direction can change and the change can be subtle at the start but drastic over time. For example, it can make a huge difference whether the goal of a health system is the best health status possible or the best medical care possible.&lt;br /&gt;&lt;br /&gt;After fifty-one years of active involvement in various aspects of the medical profession, I seek orientation and direction to understand better the basis of good health and the role medical care serves in achieving and maintaining good health. I can understand that doctors and hospitals want people to believe health status depends upon their services. Also, medical schools would be in favor of producing more doctors, as well as the hospitals and communities that want to start medical schools. Let us not forget that medicine is an industry but it is an industry oriented to sickness. Is sickness, even sickness well treated, the best pathway to better health status? Perhaps it might be better to plan on less sickness.&lt;br /&gt;&lt;br /&gt;A recent study of Texas Medicare claims data revealed a &lt;a href="http://www.medpagetoday.com/Gastroenterology/ColonCancer/30273"&gt;disparity in colonoscopy screening&lt;/a&gt; between white, black and Hispanic populations with whites receiving more screenings. In those communities with more doctors, the disparity was even greater in favor of white patients. Placing aside the question as to whether the number of colonoscopies is necessary regardless of race and ethnicity, this is one of many studies showing that more doctors frequently do not yield the desired result. That is to say the result desired by patients and the community.&lt;br /&gt;&lt;br /&gt;Meanwhile, the data continues to accumulate that better health status depends on much more than doctors and hospitals. But unhealthy lifestyles and poor diet are a plague on the individual &lt;b&gt;and society.&lt;/b&gt; It is not easy to control the situation but it can be done. The efforts in New York City schools appear to be yielding results. The number of &lt;a href="http://www.nytimes.com/schoolbook/2011/12/15/obesity-in-new-york-children-on-the-decline-officials-say/"&gt;obese NYC school children declined by 5.5 percent over the past five years&lt;/a&gt;. Other cities and towns can follow the example. Some might think it is an introduction to the transformation of medical care but it is an introduction to the transformation of society to achieve improved health status of the population and the individuals within the society. Medical care, even reformed medical care, cannot do that. It is time for the United States to wake up and accept the implications of the health outcomes resulting from a health system usurped by medical care.&lt;br /&gt;&lt;br /&gt;The United States spends more on medical care and derives less benefit from it than almost every other industrialized nation. Meanwhile, social support programs, public health and primary prevention are afterthoughts of a medical care system. What are the outcomes? The US ranks in the bottom half of comparable nations for life expectancy and infant mortality. But we are winners when it comes to obesity and chronic metabolic diseases. These problems are not cured but many of them can be prevented.&lt;br /&gt;&lt;br /&gt;&lt;a href="http://www.nytimes.com/2011/12/09/opinion/to-fix-health-care-help-the-poor.html?src=recg"&gt;An interesting viewpoint&lt;/a&gt; is presented by combining what countries spend on medical care with spending on social services, like rent subsidies, employment-training programs, unemployment benefits, old age pensions, family support and other services that can extend and improve life. In that analysis the US no longer leads among comparable nations. "America is one of only three industrialized nations to spend the majority of its health and social service budget on health care." And in the US that health care budget is overwhelmingly spent on medical care. The study revealed that countries with high health care spending relative to social spending had lower life expectancy and higher infant mortality than countries that favored social spending.&lt;br /&gt;&lt;br /&gt;Changing this situation will not be easy. Far from it. Reallocating medical care funds to social services is a start. That alone will change society. But addressing the&lt;a href="http://opinionator.blogs.nytimes.com/2011/12/08/cereal-cookies-oh-whats-the-diff/"&gt; social, commercial and political environment&lt;/a&gt; that promotes unhealthy lifestyles resulting in much of the chronic disease &lt;a href="http://blogs.scientificamerican.com/plugged-in/2011/12/07/the-impracticality-of-a-cheeseburger/?WT_mc_id=SA_CAT_ENGYSUS_20111208"&gt;requires transformation of American society&lt;/a&gt;. Generally, commercial fruit juices are very sweet and much of the nutritional value decays over a period of days after juicing. And he is willing to say that we cannot be sure about the outcomes of many proposed solutions. In this &lt;a href="http://www.nytimes.com/2011/11/08/opinion/brooks-mitt-romney-the-serious-one.html?nl=todaysheadlines&amp;amp;emc=tha212"&gt;article&lt;/a&gt; he expounds upon Mitt Romney's proposals for Social Security and Medicare. Addressing the Medicare, I agree with Mr. Brooks that the fee-for-service payment system is a central problem for medical care not just Medicare. But don't look for insurance companies to solve that or any other health problem. Health insurance is not a health care or medical care provider and it does not work like other insurance. Sharing risk is a problem when there is no differentiation between health care and medical care.&lt;br /&gt;&lt;br /&gt;No issue in the United States is more important and more misunderstood than health. The current concept of health is robbing the nation of vitality and treasure. People fear sickness more than they trust the pursuit of good health. Medical care has usurped health care and public health is relegated to a secondary role. The expansion of medical care and health (medical care) insurance has created an enormous business enterprise with the objectives and values of commerce. That warps health and health care to the detriment of health status. The goal of national health policy must be to achieve and maintain the best health status possible by reducing sickness, disease and disability. In that context medical care is the default system for those who need it, which I propose is significantly less than the current utilization. &amp;nbsp; &amp;nbsp; &lt;br /&gt;&lt;br /&gt;Now, envision a nation with better health status, more vitality and less medical care. That vision would consist of a transformed society. One of the common causes of the symptoms is regular and diet sodas. Highly processed food is another common cause. Perhaps the diagnosis should be poor diet. Those of us who worked in large public hospitals (Grady, Parkland, etc.) have seen how poverty and lack of education impact health. Education is on the national agenda and it will take time to have an impact on the problem. Other issues such as immigration will influence this effort. It makes no sense not to educate the children of immigrants, illegal or not. Even the Texans understand that. Keep in mind the physical activity does not have to be weight lifting and/or running. Walking is good. Brisk walking and walking up hills is even better. Be persistent and consistent. 10,000 steps is four or five miles depending upon the length of your stride. Most people walk one to two miles daily. KEEP MOVING! In any case, no one should ever say that weight loss is easy and those who are successful should be praised. Early detection and secondary prevention are an extension of medical care and they are just as expensive as medical care. But do they yield the expected results? This question will continue to be examined and I expect the results to show that the lives of some people are saved. Don't expect this to yield cost savings and do expect some morbidity and even mortality associated with the process of early detection and treatment. Some of the treatments will be unnecessary but that would not be obvious before the fact. My concern is that medical care, including early detection and secondary prevention, distracts from the attention and effort directed to primary prevention. Certainly, more needs to be learned about primary prevention but a workable body of knowledge is available. This &lt;a href="http://www.bloomberg.com/news/2011-10-12/health-insurers-bid-to-take-over-care-for-9-million-elderly-poor-from-u-s-.html"&gt;article&lt;/a&gt; reports on such a situation for the group of people who are recipients of both Medicare and Medicaid. The article goes on to indicate the concerns and opposition to change. Also highlighted is a more basic structural problem that sooner or later must be addressed. Why should health insurers be the ones to deal with this? They are financial people, not health care providers. But, then, they are responding to the inadequacies in the training and organization of health care providers. These issues are on the table along with proposals to correct them but none of this goes far enough to solve the basic problems - too much medical care and not enough health care, too much specialty care and not enough primary care, too much fragmentation and not enough integration. No doubt, obesity is a much more complex problem than smoking. The campaign to stop smoking has not been completely successful but it is a good start and the momentum can be helpful to obesity. Make no mistake, however, successful programs for smoking and obesity will change the community and the health system in ways we can hardly imagine. I have to believe it can be done. The defense is justified because medical care saves lives of the sick and injured. I cannot praise medical care due to its low capability to cure disease. Because we expect more from medical care than we should and neglect health care, the consequence is promotion of sickness and development of medical care as a business enterprise. But medical care is not health care. Health care would balance the health system and gradually reduce the need for medical care. The focus on health promotion and disease prevention provides a goal of good health. There are many reasons this occurred but topping the list is lack of understanding for the situation and mismanaging the problem. Health is the issue but it was ignored to focus upon sickness. The continuing and increasing focus upon sickness elevated medical care above health care to eventually and inevitably result in stalemate with chronic disease, increasing cost and deteriorating health status. Lack of understanding for health and lack of appreciation for health care surrenders lifestyle to a culture that fosters unhealthy habits and does little to promote health. The culture and unhealthy lifestyles reinforce each other and, in turn, impact the national economy, the environment and climate. Medical care reform will not relieve this situation or solve the problems but communities across the country are initiating projects to promote health at the local level. And I don't suppose I fault pharmaceutical companies for exploiting scientific research, I just wish they would discontinue advertising illness, disease and real or perceived health disorders. However, all of this influences the public mind-set to interfere with healthy behaviors and leads to unintended and dangerous consequences.&lt;br /&gt;&lt;br /&gt;The experimental drug, &lt;a href="http://www.nytimes.com/2011/08/19/science/19fat.html?src=recg"&gt;SRT-1720&lt;/a&gt;, that prolongs the life of fat mice by reducing the amount of fat in the liver and thereby increasing the sensitivity to insulin may eventually lead to useful treatments. Certainly, every industry can benefit from new thinking to address what appears to be insoluble problems. Health care is a good example. Health in the U.S. is defined in terms of sickness and medical care dominates the field. Costs spiral out of control and health status declines. The idealogues in Congress and some economists don't help by staking out extreme positions to protect or cut sick care. The dialogue has to shift from sick care to achieving and maintaining optimum health status for individuals and the population.
